Verification Worksheet

The Verification Worksheet is required by all students, who have been selected by the Department of Education for verification. Please complete all sections of this form including the parent section, if you are a dependent student. Attach a copy of your signed 2011 1040 and parent's signed 2011 1040 for dependent students to the Verification Worksheet. Include any other documentation of any other income received in the calendar year 2011.
